Annual Adolescent Physical

What To Expect:
Our practice does not perform what is commonly referred to as a Sports Physical. We will perform a Annual Physical or Well Check Exam, which will provide us all the information necessary to sign medical release forms for schools and sports teams.

Kathleen Allen, MA, LPC


Kathleen Allen, M.A., LPC, received a B.A., and M.A. in Clinical Psychology from Vanderbilt University. While in Graduate School, Kathleen worked with chronically and terminally ill young people at Vanderbilt Children's Hospital. For eight years she was head of Psycholocial Services at Chirldren's Hospital in Richmond, Virgina. Since 1982, Kathleen has worked at The Pediatric Center providing counseling and evaluative services to children and adolescents, as well as family counseling. She has done extensive post-graduate training in evaluation and treatment of learning differences and Attention Deficit Hyperactivity Disorder (ADHD). Kathleen is a member of the Licensed Professional Counselors Association of Georgia and the American Mental Health Counselors Association.

Kathleen does psycho-educational evaluations and individual/family counseling for The Learning Program as well as leading classes. She attends Student Support Team, 504, and IEP meetings. Kathleen takes referrals from physicians outside of The Pediatric Center.

FREQUENTLY ASKED QUESTIONS (F.A.Q.)

What is a Well Check?
--- This type of appointment is for a routine visit to review with the doctor your child?s growth and development. It is scheduled in advance and covers physical issues such as weight, height and immunizations as well as mental issues like school performance and behavior.

How long will I be in the office for a Well Check?
--- Usually a Well Check will involve about 1 hour of your time in the office. This is because your child will be measured and there will be forms to complete before you see the doctor. There may also be shots or lab tests after the doctor finishes. Please be aware that the appointment time we state is the time we plan for you to begin with the doctor, so get to our office about 15 minutes early to be ready on time.

How much time will I spend with the doctor?
--- Most Well Check appointments involve 15 minutes with the doctor in addition to the other actions.

Can I do the paper work before I get to the office?
--- Yes, we have our forms on our website at www.The-Pediatric-Center.com. Just scroll down from the beginning of the page to find the section on New Patient Paperwork. Print the forms, fill them out and bring them with you when you come.

Can I get forms such as the preparticipation form for sports and school forms filled out at a Well Check?
--- Yes, these can be completed at the time of your visit. Please complete the questions on the front page of the preparticipation form, because these are used by the doctor to decide on approval for sports. The school forms will be completed based on the results of the visit.

How many topics can I discuss with the doctor?
--- Since this is a routine visit, we expect that your child will be healthy and doing well in school. However, we can discuss 1 or 2 other issues of concern briefly. If a major concerns comes up, we will suggest either a Sick Visit or an Evaluation with enough time to deal properly with it.




What is a Sick Visit?
--- This is a visit which is planned in advance or scheduled on the same day to deal with issues that are of concern such as fevers, cough or wheezing. These visits are focused on one issue, and are usually briefer than a Well Check. A longer visit might occur if your child is wheezing and needs a nebulizer treatment.




What is an Evaluation?
--- This is a visit focused on a complex issue such as behavior problems, illnesses that have occurred for a long time, or school problems. These visits may take a little longer, so please tell the appointment clerk that you have a major concern.
